Cocker Spaniel-Comprehensive Breed Health Survey Project

Instructions
This survey is limited to major diseases, health disorders, and environmental considerations. These are defined as
- potentially life threatening,
- affects the quality of life for the dog, or
- affects the dogs’ capacity as a breeding animal.
Whenever possible, limit your information to conditions diagnosed by a veterinarian. If you don’t have a veterinarian diagnosis, mark the “Self” box if it is your observation alone.
It takes about 15 minutes to complete for the average Cocker. You might have your Cocker’s records at hand before you begin; such as physical measurements and weights, vaccination records, whelping records and other medical information.
This survey is confidential with all data to be collected by Larry Hopkins, Elements Software Engineering.
Please include all American Cocker Spaniels you owned who were alive on January 1, 2001 or born since.
- This does not include puppies you produced and then sold.
- This does include puppies you produced and then euthanized.
- If you obtained a Cocker Spaniel as an adult, include its health information only for the time it lived with you.
- If you co-own a Cocker Spaniel, do not duplicate information on the dog. The person with whom the Cocker lives should enter data.
- ASC members should enter dogs co-owned with non-members if the dog lives with them. If the dog lives with the non-member, then its information should be entered with the non-member’s self assigned PIN.
- If your Cocker Spaniel is with a handler (show or field) you should enter the health information for your dog.
